Physicalform.pdf
This form can be printed and taken with your student for the physical exam. Please make sure the document gets signed and turned into to the acitvities director's office or to the athletic trainer.
Medical_Consent_Form.pdf
This document must be filled out and signed by a parent/guardian. The medical consent form can be turned into the activities director's office or directly to the athletic trainer. This form can also be filled out during check-in.
RPS_Fee_Waiver_Form.pdf
Parents or students who desire to have student fees waived must complete this form. In order to verify eligibility for the fee waiver, an Application for Free and Reduced Price Meals must be completed and submitted (unless already on file with the district). Application for the fee waiver may be made at any time, but must be renewed annually. All activities for which a waiver is sought should be listed on this form.
